“SAMMS CONSOLIDATED ENTRY
FORM”
Please follow the
instructions below completely.
This will make IT much easier to enter these meets.
1) Write name as indicated, last name first, then first
name, then middle initial.
2) “LSC Code” is SN8 for Sierra Nevada 2008.
3) Club initials
are NCA.
4) Leave "If
UN Swim Club" blank.
5) Club name is Nor-Cal
Aquatics.
6) Age is the
swimmers age the first day of the meet.
7) “Date of birth”
should be self-explanatory.
8) “Amt. Paid” is
the total entry fee, including any pool surcharge.
9) Leave “USS ID”
blank.
10) Circle either M or F, whatever applies.
11) Age group will be the applicable age group as listed
on the meet sheet.
12) List the event number of the events you want to
participate, in the boxes under “EVENT”. Please list those in the order
that is listed on the meet sheet.
13) Leave ENTRY TIME blank. The coaches will fill
in the times.
14) List the corresponding stroke and distance you are
swimming in the boxes under STROKE, i.e. 100 Free, 50 Breast, etc.
15) In the COACH box, you can put my name, Brian
Wilson.
16) Put your own address next to SWIMMER'S ADDRESS.
17) Put your phone number next to PHONE.
Any incomplete or
incorrect forms will be returned to you for correction. If you have any
question at all, please ask me or any of the coaching staff.
